I'm answering this story from the perspective of Christian theology, because otherwise it doesn't make a lot of sense to even ask the question.
The answer is no.
The Bible reports at least two people (Enoch and Elijah) in the Old Testament who were taken bodily into heaven without dying. Also, Jesus tells a parable at one point about a beggar and a rich man who have died; the beggar winds up with Abraham, and the rich man winds up in a place of torment. The important feature here is that wherever Abraham and the beggar are in this story, it's not hell (the rich man's location is not precisely specified; it's not clear if he's in "hell" proper or some kind of "pre-hell"), and Abraham predates Jesus even if the beggar does not.
Based on these known exceptions, it's reasonable to assume that there are probably others as well.
The usual Christian theology holds that Christ paid the price for past sins as well as future ones, and all those who "trusted God" during their lives were redeemed even if they had already died by the time Christ was born. Romans and Galatians both contain passages to this effect.

Absolutely not. How unfair for the millions/billions that would be and God is fair. Jesus Himself, explained that those before His time would stand in the Judgement process and given their opportunity:

What people have misunderstood that people burn forever and ever, but that's not Biblical. What everyone thinks "Hell" is not going to happen until the Final Judgement where everyone who rejected Christ will receive "Hell"; they will burn according to what they knew and how they decided to make their decision.
