There are no additional side effects (other than the ones caused by the individual medications) to using the combination of phentermine and birth control pills. Also, there are no drug interactions between them. There is no evidence to suggest that the use of phentermine makes oral contraceptives (birth control pills) less effective.
Like all medication, when it expires, it becomes less effective (and increases in its ineffectiveness as the time goes on). It's not easy to give a percentage as to how effective compared to non-expired pills, but if using expired birth control is your only option, I would use it with a condom or some other non-hormonal birth control to make sure you don't get pregnant. Otherwise, the likelihood of pregnancy will increase.

There shouldn't be any adverse effects to you as the user. The fact that it's expired simply means that it will likely be less effective as a lice killer or control. That said, you probably shouldn't use it. For the simple reason that, like the old adage that says, "What doesn't kill you, makes you stronger," the lower efficacy of the expired product may allow some of the lice to, not only survive, but become resistant to the shampoo. Once they become resistant to it, the full strength shampoo will be less effective at controlling the resistant lice as well. Take the expired shampoo back to your local pharmacy for disposal.
