At least in the United States, it's not actually legally required. However, the custom is so entrenched that you can expect people to be confused if you don't, and further expect them to call you by his surname even if you try to keep your maiden name.

Answer

Beware of archaic and biased advice. In Western culture it is extremely common for women to maintain their own names when they marry. There is no legal requirement for a woman to change her name. It is becoming more and more common and is a good idea especially with the high divorce rates. Modern culture no longer requires that a woman surrender her identity when she marries. The rate of women retaining their own names is higher in more educated populations especially since there are so many female professionals represented throughout the various professions. They do not generally change their names once they have established their business and professional identities. It is not confusing at all. In fact, it shows we are still evolving and making progress.

Is a marriage legal if a marriage certificate is not issued?

That would depend upon the laws of the state in which the union occurred. However, state laws require that the person who can perform legal marriages must issue a certificate of marriage to the couple after the ceremony has been performed. A copy of the certificate is then made a part of public record. Certain government agencies such as the Social Security Administration require a certificate of marriage before they will change the woman's name on SS records (assuming she wants to take her husband's surname). A marriage license is not acceptable proof that the marriage has taken place.
