The failure rate for tubal ligation (having your tubes tied) is approximately 0.4%, and the failure rate for vasectomy is approximately 0.15%

Therefore, the chance of pregnancy with both methods being used at the same time is 0.4% x 0.15% = 0.0006%

So very unlikely!
