Because the uterus is swollen, the incision site must be larger. Since the hormones are raging as well, the risks involved with surgery are somewhat increased.
Ideally, it is best to spay a cat when it is not in heat. A skilled veterinarian can minimize the risks and safely perform a spay while a cat is in heat and even when a cat is in advanced pregnancy.

Yes. Just do it! Female cats go into heat every three weeks from February to the end of October as a general rule. They only release their eggs from the ovaries when they are bred by a male cat (they are sporadic ovulators), so they'll drive you crazy and want to be outside to get bred all those months. Actually finding the uterine horns is much easier when they are in heat and they seem to do just fine just about any time of their heat cycle.

It is never safe to interrupt an hormonal process as it may cause imbalance in diverse related functions and systems some of which may manifest later in life. Wait until the cycle has finished. The best time is one week after.
