It's a real TV show certainly and the fact it is a TV show designed to entertainment, it's entirely possible while everything is 'real' it could be staged realism for the camera.

I believe it's real. What makes you think that? Is it because he never gets any fatter lol. He exercises everyday and the day before a challenge he doesn't eat, just drinks lots of water
