Sailor Cosmos is the Ultimate form of Sailor Moon (in the manga) ---- (Manga): Sailor Cosmos is the last incarnation and the ultimate form of Sailor Moon. During her time, there was a great war between her and the good and Chaos. When she couldn't stand the pain any longer, she went back into the past as Chibi Chibi, AKA Sailor Chibi Chibi Moon to help Eternal Sailor Moon defeat Chaos. She reveals herself later in the story, calling herself a coward. (Anime): Sailor Cosmos is the ultimate form of Sailor Chibi Chibi Moon, who is actually Galaxia's starseed. Unfortunately, we never get to see her on here.:(

It's debatable. Sailor Cosmos only appears in the manga, and when she appears, she doesn't specifically say who she is. However, one of Sailor Chibimoon's guardians, Sailor Ceres, says "Don't tell me you're...Sailor Moon's future ultimate form?!" (Sailor Moon vol. 12, Kodansha English edition, p. 243). This would imply that Cosmos is an evolution of Sailor Moon, similar to how Super Sailor Moon or Eternal Sailor Moon is an "evolved" or "leveled up" version of Sailor Moon in prior story arcs, or similar to how Usagi/Sailor Moon was formerly known as Serenity, and Cosmos is a future incarnation of Sailor Moon.

However, the wording is vague enough to allow room for alternative interpretations, including the idea of Sailor Moon as a title, rather than a specific person--that is, once Usagi becomes Neo-Queen Serenity, she ceases to be Sailor Moon and Chibiusa takes that mantle, changing her Sailor Senshi name from Sailor Chibimoon to Sailor Moon. That could mean that Sailor Cosmos is a descendant or future bearer of the title and powers of "Sailor Moon," but there is no timeline given as to when she is from.

However, in Kodansha's English translation, Cosmos responds to Ceres by discussing the destruction Chaos wreaked in her timeline, and remembering her battle "here." Cosmos says "...And [I'd] think to myself, 'if I'd just obliterated the Cauldron back then, could war and suffering have been eradicated...?'" This seems to imply that Cosmos is, in fact, a future form of Usagi as Sailor Moon, rather than just a holder of Sailor Moon's powers, evolved through history. She indicates that she came to this point in Time-Space to cheer "the me of this era" up and guide her down what she deemed the right path: destroying Chaos at the Cauldron (p. 247).

In Sailor Moon, only one timeline seems to exist continuously, but changes in the past take time before they ripple into the future, which is how Cosmos continues to exist despite Sailor Moon having taken an alternative path toward ending the battle with Chaos than what Cosmos herself did in the past, or what Cosmos wanted Sailor Moon to do. Further, if Cosmos is a reincarnation of Sailor Moon/Serenity, then it is possible she would still refer to Usagi/Sailor Moon as "the me of this era," like a past self, but not be affected by Sailor Moon's choices to the point of her own existence becoming endangered.
