Generally, no, there isn't a "form" to disclaim a portion of an estate that was left to you. Your best course of action is to contact the executor of the will and/or the court that adjudicated the issues of the estate. A personal letter signed by you may get you off the "guest list" as regards an inheritance, but it may not, too, depending on the terms and conditions of the will and the laws in effect in the jurisdiction in which the will went through probate (the validation of a will or other document).

The bottom line is that you'll have to find out who wants to give you the assets you were left by the departed and talk to them directly. You should be able to do this (talk to the executor or the court) without an attorney, but a strong suggestion is made that you get counsel on this. An initial consult is usually free or quite inexpensive (because the lawyer is hoping to land you as a client), and there's almost no substitute for professional advice. Be aware of your options and chose wisely, being sure to make the decision with your head and not your heart or other things.
