Not physically, LSD is a very safe substance if you only look at physical damage. No long term physical effects have been reported in it's use, even heavy frequent use. The lethal dose of LSD is thousands of "hits", and only one person has ever died from it. The one person that died took the equivalent of thousands of street doses of LSD intravenously, this was not done recreationally, but was administered to an unknowing volunteer by the American army...

That being said, LSD can be very risky psychologicaly, people with existing mental conditions may find them worsened, sometimes for long periods of time.

People with schitzophrenia may set off intense symptoms, and might permanently worsten their condition.

Even perfectly healthy individuals may find themselves depressed, end up with flashbacks, or even mild psychoses after a difficult experience.

LSD is a very powerfull drug, that can completely change the way you mind works while your high, and that can have lasting repercussions. Most tales of insanity caused by acid are false, it's very rare for persistant problems to happen, but they can. LSD can be a very powerfull tool for self discovery, spirituality, and psychotherapy, but the very things that make it such a powerfull tool, can also make it dangerous. It IS possible to change the way you think, permanantly, for good or for ill, and LSD should always be treated with respect, and aproached with caution. Set and setting are very important, the way you feel, who's arround you, where you are.

What is the largest dose of LSD someone has ever taken?

The largest voluntary dose of LSD ever taken was 750,000ug (7,500 street doses) in 1994. The individual suffered from residual effects for some time, though evidence of permanent damage was insufficient. The largest accidental overdose was a user who had mistakenly injected 320,000ug of LSD under the false conception that it was methamphetamine. http://www.erowid.org/references/refs_view.php?ID=1389
