Lower bowel pain dry mouth cold-sweat and extreme weakness?

I can't say much except that I've experienced this about once a year in frequency. It does NOT appear to me to be food poisoning (as that is generally, in my experience, accompanied by a great deal of gurgling, burbling and sudden evacuation of the bowel and probably stomach about 4-6 hours after eating). This occurs when sitting on the toilet and while I feel like I will pass a stool I do not - then a DEEP DULL ache, just at the base of my spine *inside* followed by progressive intensity in pain, nausea, salivation (which happens because of the nausea no doubt) and at this point I cannot sit and basically crawl to the floor - here cold sweat basically POURS out of me, chills. After this passes, about 5 minutes, I can stand up and usually am "OK".

Now this suggests constipation but I cannot possibly believe constipation would be THE worst pain I've EVER felt in my 43 years of life. It's been happening now for about 7 years, 9-12 months apart (I'm male btw so it isn't a baby either). Further my diet is by far and away ABOVE average for fiber, cholesteral is good, health over all is excellent, save stress is a definitely higher than average (but blood pressure, prostate health is fine too). No cancer, no HIV, etc. and no history of abdominal pains like this (some acid issues for my father, but that's it, oh and he did have gallstone removed).

I eat fruit and oatmeal and whole grain bread only. Not a lot of meat, what I do is low in fat. So this is all a mystery.... Doctor said it was diet related - but how can it happen so infrequently???
