For the greatest number of individual kills, most sources cite Ogami Itto (played by Tomisaburo Wakayama) as the character to have killed the most people in one film - 'Lone Wolf and Cub: White Heaven and Hell' (also known as 'Shogun Assassin: Cold Road to Hell') with 150 kills. Apparently Smith (Clive Owen) in 'Shoot 'em Up' is second with 141. This excludes films where whole planets or cities are destroyed as there is no possibility of making an exact body count.

The above two place 2nd and 3rd respectively, and Christian Bale places 4th with 136 kills in Equilibrium.
