To make sure that your sim will have twins you have to have 10,000 lifetime wishes and for 10,000 lifetime wishes it will say fertility treatment you purchase that lifetime wish reward and you might have 2 or 3 babies and its very rare but you can have 4.

∙ 12y agoI'm not sure if there is a cheat or a special food you can eat that will give you a higher chance for twins.
I've heard that is you eat watermellon you can have a girl and apples for boy. Also when I play on the wii as soon as you get home from the hospital click try for baby and right after sometimes they'll be taken to hospital to have another baby.
