Yes a passport is required, unless you are a US or Canadian citizen and cross the border via land, i.e. driving across, train, bus, walk, etc. In the case of crossing by land, a valid government issued photo ID accompanied by a birth certificate (the one you get from vital statistics, not the one with the baby feet inkprints on it) will be good enough to establish proof of citizenship until 2008 at which time tentatively passports will be required to enter the US by any means.
