Damage to anything covered by the landlord's insurance, because the landlord owns the property not the renter. Such damage would be covered under homeowner's insurance, because the homeowner owns the property.

I can only speak for California, and in Ca we do not ask for proof of insurance. It's up to the renter to know what they have, and do not have. In Ca the renter is responsible for any damage regardless of fault.

The landlord is usually only responsible for negligent damage, not fire or disaster. You can buy renter's insurance to cover your furnishings.
It would depend on the state the accident was in. State laws would determine who is responsible. in California the renter is responsible regardless of fault. Also if you accept the LDW (Loss Damage Waiver) and you violate the contract the LDW/CDW is void. So if an unauthorized driver was driving the car and got into an accident, the renter would still be responsible.
