From what I've heard, if you beat Sonic R and play as him, he appears. But, I you have a flash light in the bathroom, your alone and in the dark, listen to Sonic R's "Living in the City" backwards, wait and you see a red light, like the one on the tails dolls head. But, the curse is that it will come and either kill you, or steal your soul.

As far as I know, the Tails Doll will curse you with either a soul-less life or no life at all. He'll either kill you or steal your soul and let you live a meaningless life.
