currently Imam Mahdi who is dissappeared but some especial Shia Muslims meet him and he guids them.

recently Imam Khomeini was leader of Iran Islamic revolution who was main shia leader in world. and after his death recently now Imam Khamenei is leader of Shia Islam in Iran and also shia countries.

other local shia leaders exist in shia countries but Imam Khamanei is main leader in shia world.

There are numerous types of religious leaders in Shiite Islam. The most famous and most authoritative are the Ayatollahs who are responsible for all Shiites in a certain geographic area and have no superiors (similar to Orthodox Patriarchs).

According to Shia Islam only one of the fourteen infallibles can be leader of humans of the world. currently the 14th of them (Imam Mahdi a.s.) is at occultation because Muslims killed all of 11 infallibles Imams before him. but at occultation he still supports and guides belivers but secretly and not publically. according to his sayings at times of occultation an expert Islamic scholar with some specific terms should lead the Islamic society. currently Imam Khamenei is agreed by shia Islam scholars as leader based on guidelines of Imam Mahdi a.s. for leader. for more information you can refer to below books:
