Vital signs are typically considered to be Heart Rate, Blood Pressure, Temperature, Respiratory rate and some consider pulse ox as the last. Any reading outside of these ranges would be abnormal. Normal Range (Adult) Heart Rate 60-100 Blood Pressure 120/80 Temperature 98.6 degrees Fahrenheit, 37 degrees Celsius Respiratory rate 12-20 breaths/minute Pulse Ox >95%

history of disease eg. congestive heart failure, myocardial infarct

difficulty in breathing, can be caused by congestive heart failure as well, most common cause in fact

I think it's when your vital signs (or your heartbeat) isn't normal. Example: Your normal vital sign 50 beats per minute and you get it checked and it's 30 beats or maybe 70 beats per minute.
