When piercing the tongue, the risks can be hitting a rather large blood vessel and bleeding out.

However an experienced piercer would check the bottom of your tongue to see if there are any major veins before positioning the piercing.
Also if you have a relatively small tongue, once it is pierced, the jewelry can irritate the frenum and gums.
