This is a simple question, but in some cases, simple questions do not have a simple answer. On September 8, 2010, BP posted to their website a report of their internal investigation of the accident. (see related link) Two companies, Halliburton and Transocean, who had employees working on the rig the day of the accident have issued press releases critical of the BP report.

Also, there are a number of ongoing investigations. The US Coast Guard and the Marine Management Service are conducting the "Deepwater Horizon Joint Investigation" (see related link) and President Obama has commissioned a special task force to review the event and deepwater drilling in general, and make recommendations.
The spill was caused by a well blow out, or an uncontrollable flow of oil. The oil flowed from a oil deposit approximately 18,000 ft below sea level, up the well and out into the Gulf of Mexico.
The blow out was caused by a two failures. First, the well barriers such as cement designed to stop the oil from flowing into the well failed. Second, the blow out preventers did not work. The blow out preventers are emergency well shut in devices located on the sea floor.
If the well barriers or the blow out preventer had worked, there would be no spill.
It is a bit more complicated than this. The BP report discusses eight key findings or contributing factors that lead to the fire and explosions aboard the Deepwater Horizon. I believe all investigations will conclude that a series of events caused the accident.
