What causes oil to blow out of the oil filler cap?

A. i work in a desil repair shop, big rigs, so im not sure if its the same but where i work its a big problem, usually needs an in frame kit. the problem is the compression in the cylender(s) escapes somehow (rings, scratches in the cylender wall ect.) into the crank case, in turn pushing oil through the filler!,
