There are two island countries that are surrounded by the Mediterranean Sea, Cyprus and Malta.

The Mediterranean Sea (including the Adriatic Sea) is bordered by a further 17 countries:

Spain, France, Italy, Slovenia, Croatia, Bosnia, Montenegro, Albania, Greece, Turkey, Syria, Lebanon, Egypt, Libya, Tunisia, Algeria and Morocco.

Some of these countries include Islands in the Mediterranean Sea, for example:

Sicily (Italy), Corsica (France), Sardinia (Italy), Majorca (Spain).

& technically the best answer is perhaps only the republic of Malta

since it is the only country that is fully & directly surrounded by the mediterranean sea itself

while still more technically

perhaps even Malta is not entirely in the sea

since it is of course first & foremost on dry land

yet certainly the republic of Cyprus too is fully surrounded by the mediterranean sea

tho not directly by the mediterranean sea alone

& similarly the republic of northern Cyprus

but only if it is considered a separate country

so

there you have perhaps 1 or 2 or 3 countries

but in addition

nearly all the many countries that directly surround the mediterranean include at least a few minor islands within it

but most notably Spain France Italy Croatia & Greece

& so to that extent they too as well as others could be said to be in the mediterranean sea

tho such countries as these are far more often said to be on it than in it

& finally

as well as perhaps least arguably

the United Kingdom may also be considered to be in the mediterranean sea by virtue of the sovereign territory it holds on the island of Cyprus
