At the Last Supper, Jesus said to His disciples, "Do this in remembrance of me." He was referring to communion. The wine (or Grape Juice) is His blood, shed for us. The bread is his body, broken for us.

Jesus knew his time on Earth was short, and the Holy Prophecy was at hand. Therefore, he gave his disciples the Holy Sacrament, symbolic of his body and his blood. It is a covenant for us to be "Christ-like". So when he says, "Do this in memory of me", he's telling them (and all of us) to be Christ-like.

1 Cor. 11:23-26: We partake of the Bread (Body) and the Cup (New Covenant effected by His blood) in remembrance of Christ. Depending on your particular Christian tradition (Eastern Orthodoxy, Roman Catholicism or Protestantism), it is known as either the Eucharist, Communion or the Lord's Supper.
