The law in the US (since I don't know what country you are in I'm assuming here) says that you can get one at any age, it's only the female carrying the embryo that is allowed to make the decision regardless of her age. In most states they have a demand on parental consent or notice if you are a minor. The girl can then get a waiver from the court if she does not wish her parents to know.

Abortion is always legal for rape and incest and health reasons. On demand the limit is up to the individual states and they have set it at 20 or 24 weeks. Abortion is not legal on demand past week 24. Past week 24 it's premature labor. In case of rape and incest it does not count as health reason past 24 weeks. Health reasons past week 24 means when the mothers life is at risk or the baby will not survive or is already dead. If the mothers health is at risk past 24 weeks they will deliver the baby in premature labor and try to save it.
