They do realize that some individuals have an astonishing ability to understand and manipulate human relationships, to read body language at a glance, to recognize an emotional disturbance from the ripple effects on third parties, and even to create a disturbance in the center by creating coordinated ripples. This is what the "master manipulator", Charles Manson, cult leader, type can do.
This is just the relationship thread mental architecture, which turns the whole brain into a relationship computer, and does non-verbal processing of body language. "Stringers" who manipulate heart strings.
Shrinks do know that some people are amazingly perceptive, and sometimes seem to read minds. This is just non verbal integration of micro clues, but it passes for telepathy too.
There are lots of mental architectures that give the person an alternate reality, some of which are valid and work well for the individual, and some of which are broken and are just mental distorders.
