In an evaluation is basically a summary that evaluates and asseses work. An evaluation of your own work is where you write how well you think you did, what you did and what you could improve on.
To evaluate is to analyze all available information and decide on the final value(s) or worth of an article, a book, an idea, a recipe, an employee's job performance, a job applicant etc.
