It depends on who you talk to. Some believe it to be a reference to an afterlife where those who were most wealthy or natural leaders would be "last" after their natural life and the more humble people would be "first" in the afterlife. If you talk to others it is just considered to be the ramblings of the old men who were writing The Bible and feared their own mortality.

It means that if you think that you are perfect and everyone loves you sooner or later you will be forgotten and put behind people, but if you are always thought about last and no one ever notices you unless they want or need something sooner or later people will start to realize everything you've done and they will start thanking you.
