The Doctor means to plant a seed of doubt about Harriet Jones' ability to lead her people, amongst the inhabitants of Earth. With such a simple phrase as "Don't you think she looks tired" he manages to plant that seed by implying that all the stress of the position seems to be getting to her and she may in fact not be fit to lead after all. The rumor of her not being able to handle the job spreads quickly and ultimately ends with her no longer being Prime Minister.
Also in a way it became a self fulfilling prophecy, with Harriet panicking and worrying about what the Doctor said, which may well have indeed made her look tired and be unable to do her job.
