It is supposed to be an insult. Questioning someone's paternal origins is implying that their mother had so many partners that she doesn't know who their father is.
Lately, it's become more of a, "Who's your daddy? I'm your daddy and I just spanked you," kind of thing. More along the lines of the first answer to your question.

Actually, the term "Who's your daddy?" is often said to women when a man is 1) proud of himself and wants to boost his ego, 2) is a provider, gift giver or sugar daddy and/or 3) on the sexual prowl.

I've only seen this phrase used in movies, and never in real life, fortunately. In movies it always seems to mean "Who is satisfying you sexually (right now, at or near the moment of climax)?" What an odd way to say that! Maybe a lot of screenwriters were abused by their fathers.
