It stands for a "hold", a stat created in 1986 by John Dewan and Mike O'Donnell (though it was earlier conceptualized by Bill James) to measure the effectiveness of middle relievers. A hold is awarded for any pitcher who enters the game with a lead, records at least one out, and leaves the game having never given up the lead. (James' definition required the reliever to pitch at least one inning.)
