The first time you're caught, you lose your CDL for three months. You'll also lose your job, and the reason (CDL suspended due to positive drug test) is recorded on your DAC report if you're with a company that is a DAC member. Any other trucking company that subscribes to DAC is going to see it.
The second time, you lose it for a year. The third time, you can never get another CDL.
The DOT's CDL drug test is for drugs truckers typically abuse: pot, cocaine, opiates, amphetamine and methamphetamine. They don't test for downers because no trucker would ever use downers--if you're going to take drugs you want drugs that will keep you awake, not ones that will put you to sleep.

However, anyone who has to drive a vehicle (delivery drivers, truck drivers, managers, outside sales people, etc) do have to take a drug test (not sure what type) as "part of DOT regulations".
