a Medical Assistant, there are a lot that can contribute to an inaccurate vital sign reading. And some of the reasons are not knowing the right way to take vital sign or not following the right procedure.For example: Respiration. As we was thought, respiration is done by counting one full minute as the chest raises and falls, which is one inhalation and one expiration. However if we as healthcare worker don't follow the rule but rather count the inhalation as one and expiration as another one, it will lead to an inaccurate vital sound. Another fault can be the Pulse. Take for instant; if we are told to take a patient pulse and we didn't do it right it can lead to false reading.

Errors introduced by a Medical Assistant are so many and it keeps on happening in the medical field. Although, there are changes been made to eliminate it. Nevertheless, error are still been made. Error that can be introduced by a medical assistant are mostly done when entering patient's information in the EMR either by misspelling the name, wrong DOB, medical abbreviation, Drug Administration or when during Vital sign.

However medical Assistant can reduce errors when taking vital signs by being accurate as possible, because it is important to do the right procedure on the right patient to get the right and satisfying results. Another reason why medical assistant should be more accurate is because the doctor diagnose and treat patient's based on what the medical assistant give them and from the lab result. So in order to avoid errors we as medical assistant should be more attentive to our patient and procedure to get an accurate vital sign or order procedure. We should also ask the doctor for clarity if we do not understand what he wrote or said, or ask for help from the next MA if we didn't heard the blood pressure clearly or get the pulse right to avoid errors.

Drinking, Smoking, Lack of Excersire are all factors of innacurate vital signs and certainly affects them aswell
