If your child is traveling with someone who is not their parent, maybe a babysitter or relative, and is leaving the country, they need a notarized travel authorization.
A statement of consent form for your child to travel
Yes if you are to travel by air but if it is by sea of land, no need but you need a proof of identification and the passport is the best one you have for the child.
no
As long as it is domestic travel (within the U.S.) children do not need ID as long as they are traveling with an adult who can present ID. If the child is traveling alone and does not have an adult who can present a valid ID at check-in, then the child will need to provide ID. Any Government issued form of ID should suffice. I would recommend checking with the airline to be sure. This answer applies to air travel only.
If you are going on a cruise from a US port and the child is a US citizen, you will only need a state-issued copy of the child's birth certificate. If you are flying, the child will need a passport.
No but they need a passport if they live in Canada.
No of course not. You don't need a passport to travel within one federal country!
i am 90% sure it is 8
Child is the full form of child.

No - ID will be OK - but it is sensible to carry with you anyway
No, actually all you need is some form of official ID.
