Malnutrition would only lead to birth defects if she was pregnant. I don't think it would stop her from getting pregnant. Irregular periods are also no reason for her to not be pregnant, although it could be confusing trying to determine when her last period was.
As for being punched in the stomach, if it was prior to her having sex then she should be fine. The body is amazingly resilient, and she would have noticed any damage done by now.
I'd have her take an at-home pregnancy test or make an appointment with the doc.
