It means you pee your pants and you've held it too long. You can get bladder infections, and you can pee blood. IT'S NOT GOOD!

Also, if your pants were already down, but you weren't at the toilet yet, you pee where ever you are. Wanna save water? Pee in the sink, bathtub/shower or outside.

No, unless you were to be sliced apart in something like a car crash.

The bladder has 2 valves called sphincters. One is controlled automatically, the other voluntarily. If neither were to open, the fluid would back up the ureters (the supply tubes from the kidneys) to the kidneys and cause plenty of damage. But unless you have an illness, a valve should open long before this would happen.

no, you bladder only fills to the capacity of about 3/4 full. then it will send an electrical signal to the brain and then the brain will tell you that you need to pee/urinate.

No. If there was a health reason preventing fluid from leaving the bladder via either of the two valves, the fluid would back up all the way to the kidneys and cause plenty of damage.

Has anyone ever exploded from a full bladder?

No. Bladders don't explode. The tissue structure of this organ is extremely flexible, and simply stretches as needed to accommodate the body's waste byproducts until 'elimination' (urination). For an explosion to occur in nonexplosive material, an abrupt extreme increase in pressure is required in an enclosure not suitable for that rapid pressure increase. The bladder does not meet either of these conditions. There is no natural method of 'abruptly increasing pressure' in the bladder to a point of explosion. Any pressure on the bladder that reaches an 'unbearable' level will usually result in the person urinating to relieve the pressure (often referred to as 'wetting' your pants). A bladder can rupture though.

What is continuous bladder irrigation?

This is a sterile procedure where a 3 way catheter is placed inside the bladder. One port allows the instillation of saline continuously. The other port allows the fluid to drain back out. This is usually done when there is bleeding from the bladder either from damage, disease or procedures. If the irrigation isn't done, clots can form inside the bladder causing the blockage of urine. It is usually not uncomfortable, in fact brings relief to some that have been unable to drain urine due the blockage.
