In terms of overall mortality rate, historical mortality totals, virulence, and negative societal impact, you would probably have to answer smallpox. In the new world ( north/south america) native populations suffered waves of smallpox epidemics responsible for huge total mortality (perhaps 50%). Asia, Europe, Africa, and the sub continent experienced similar waves of infection, although natural immunities, to varying degrees, protected these populations somewhat better. This viral disease no longer exists except in storage, kept there by Russian and American scientists reluctant to destroy the last known examples.

Well to be completely blunt without confusing you. Something as simple as a splinter that does not get removed can cause death. The reason is that it causes a systemic infection which then if left untreated will cause your body to basically become poisoned. To put it simply, you can die from nearly any illness if left untreated medically!
