Wikipedia states that for back pain:

acute = less than 4 weeks

subacute = 4 - 12 weeks

chronic = greater than 12 weeks

Note that some pain felt in the back may not be as a result of damage to the spinal tissue. Other potential sources of back pain include infection (including kidney infection), rib damage, pregnancy, cancer, and inflammatory disease.

Acute pain is when the pain comes as a result of injury and relates only to that instance. For example hitting your thumb with a hammer would result in Acute Pain (it will go away). Chronic Pain is a result of illness or injury whereby the pay does not go away. For example Arthritus will result in chronic pain because the pain is a part of the illness. Chronic Pain is part of the condition and not a side effect of the injury or illness.
