A "Form C" contact refers to a relay (or switch) contact arrangement. Form A Single pole single throw normally open (de-energized state) SPST-NO. Form B, is the same thing, but normally closed SPST-NC. A form C is a single pole double throw,SPDT, where the common pin is normally connected to one of the contacts (de-energized), and then flips over to the other contact upon energizing the coil.
