Alkaline phosphatase removes 5' phosphate groups from DNA and RNA. It will also remove phosphates from nucleotides and proteins. These enzymes are most active at alkaline pH of around 10.5- hence the name.

Alkaline phosphatase is a liver manufactured enzyme that is important to the body. When the body is deficient in alkaline phosphatase the patient may experience shortness of breath, fatigue, constipation, cold intolerance, weight-loss, and rapid heart rate. This deficiency may be caused by hypophosphatasia (a rare genetic disorder), malnutrition, b12 deficiency, Wilson's disease, Chronic Myelogenous Leukemia, menopause, hypothyroidism, anemia, and aplastic anemia. The deficiency can be treated by first finding the cause of the deficiency , then by giving the patient vitamin supplements, and a change in diet to foods high in phosphate, healthy fats, zinc, vitamin A, and vitamin B12.

Elevated alkaline phosphatase is a condition where the levels of alkaline phosphatase exceed that found in a reference range. It can be associated with certain medical conditions.[1] or syndromes (e.g. Hyperphosphatasia with mental retardation syndrome, HPMRS)

If it is unclear why alkaline phosphatase is elevated, isoenzyme studies using electrophoresis can confirm the source of the ALP. Heat stability also distinguishes bone and liver isoenzymes ("bone burns, liver lasts"). Source Wikipedia.)
