"Hola como estas" means "Hello how are you", depending on your mood you can answer in different ways.

Mal = bad

Mas o menos = alright, so so

Bien = fine

magnifico = great

Now you can say "muy" in front of any of these words to place emphasis on it. For example, if you felt really bad you would say "muy mal".
