It depends on the credit card. There can be up to 19 digits in a credit card; most issuing agencies do not use the full 19 (16 is pretty typical; American Express uses 15 and Diners Club uses 14). The first digit specifies the type of card; the first six specify the issuing institution. The last digit is a "check digit" calculated based on the others, so someone who tries to make up a number at random will get it wrong nine times out of ten.
Other than that, there really aren't any restrictions on what numbers are "possible" (note that "possible" is not the same as "valid" ... if you make up an issuer or account number that doesn't exist, the card will not go through even though it technically meets the specifications). POS terminals and data communications are cheap enough now that it's fairly common for transactions to be authorized at the time of purchase rather than later in a batch process, as was usually done 30 or 40 years ago.
