There are many ADA compliance regulation for the workplace. In general all first aid kits and right to know literature must be easily accessible to all. The suggested mounting height is 48 inches.
ADA counter height cannot exceed 34" from finished floor. ADA dishwasher height is around 32.35" from floor, add to that a 1.5" thick counter and you are right at 34".
According to ANSI 156.19, automatic ADA swing door operater push plates are to be mounted at a minimum height of 34" on center above finished floor, and a maximum height of 48" on center above finished floor.

Commercial doors are prepped for locks with the lever or knob height at 40 5/16" above the finished floor (AFF). One commercial door company height is 38"AFF. At no time can you have the lever or knob more than 48" AFF. It will not meet the ADA requirements if it is that high.

usually it will be placed according to request because many elderly or handicapped have different situations. the grab bar can be placed horizontally, diagonally, or vertically-- and the height can start from about 28 inches off the floor or higher for taller persons. ADA Horz bars are 34 to 36" to the centerline of the bar. If you are mounting in an ADA location you have to get it right. The commode and bars have to go where they have to go. Do your research.
