Go to www.profilerehab.com and browse the layouts until you find the type of layout you want to make yourself. Take the code from the layout you like and then replace the urls of each of the images to your own. Just save each image first and then make images of the same size with your own design, upload your images to photobucket(or a similar site) and then replace the urls and put it into your profile.
