"Infantilism" usually refers to paraphilic infantilism, the desire to wear diapers and be treated as a baby.
In his 1967 book, "A Textbook of Psychosexual Disorders," Clifford Allen mentioned a 1910 case described by Petlow. However, the diagnostic criteria for paraphilic infantilism would not become formally established until the publication of DSM IIIR in 1987.
Since these criteria included clinically significant distress or impairment, many AB/DLs (Adult Babies and/or diaper Lovers) have this desire, but do not meet these criteria. The intent of these criteria is to separate those who might benefit from treatment from those who would not. Most AB/DLs would not benefit from treatment. It is expected that DSM 5 will redefine paraphilic infantilism to include those who don't experience clinically significant distress or impairment.
Given that infantilism is a variation or deviation, it is meaningless to try to define "true infantilism" versus "false infantilism."
