The witch in Disney's animated classic The Sword and the Stone is called Mad Madam Mim. Her character enters the story toward the end of young Arthur's education by Merlin when he has been turned into a sparrow to learn how birds fly. Being chased by a hawk, Arthur nicknamed "Wart", flies into a chimney in the forest which lands him smack in the middle of Mad Madam Mim cottage. Merlin arrives just in time to save Arthur from being eaten by Mim. The two wizards now compete in a wizards duel that is true Disney animation at its best. Merlin wins by turning himself into a germ that Mim ingests causing her to become sick. These two scenes, with Arthur in the cottage and the wizards duel, are not that long but Mad Madam Mim is so wonderfully animated that her character has become one of the favorite Disney villains.

The sword, as far as I know, had no name. Excalibur is a totally different sword. Arthur got Excalibur soon after breaking the sword in the stone in battle.

He did pull a sword out of stone. It wasn't the same sword King Aurthur pulled. Sir Galahad's sword didn't have a name.
