While a pharmacist can provide information, the correct dose is purely a prescriber's role. And the prescriber will make that evaluation based on trial and error, starting with a low dose and working up, and maybe even changing medications entirely. I am no expert, but I have been motivated to learn, to ask a lot of questions of a number of medical providers who focus on ADHD in either or both kids and adults, and to do some independent research. I have spent hours researching drug manufacturers' web sites and "Googled" web sites on the topic. There is a broad consensus on a point I find remarkably counter-intuitive -- The "correct dose" for a person is essentially unrelated to age, height, weight, body fat, gender, race, or general physical health, although drug-interactions and other mental illnesses are a MAJOR concern - mixing Concerta with some drugs is very dangerous, and ADHD meds can complicate some Bi-Polar conditions, among many other risks. Concerta has been evaluated for a "Black Box" warning, but I do not think one is required at this time. The point is, the prescriber needs a lot of information about the patient to safely prescribe any dosage. And weight and body fat are not information of any particular importance.

Blood volume, brain chemistry and metabolism are the basic determining factors for what dose is ineffective, effective or "high".

Concerta is available in 18mg, 27mg, 36mg, 54mg and 72mg extended release capsules.

18mg is the common starting dose for children and 18mg to 36mg for adults depending on discretion of the doctor.

The maximum dosage of Concerta should not be greater than 0.9 mg per pound of body weight.

For people switching to Concerta from Ritalin, the dosage requires careful consideration because 18mg of Concerta is much different then 18mg of Ritalin due to Concerta's precision time release mechanism.
