Is your grandson the child of a child you and the person who is the Great grandparent of the step great grand daughter? If so, they would be first cousins once removed, sharing a common ancestor. Otherwise, they are not related.

Let's say that grandma had a daughter, and the daughter had two sons, each with a different man (or grandma had a son, who had sons with two different women). The boys would be half brothers, and she would be the grandmother of both.
If grandma had a daughter who had a son, then his father had a son with another woman, the boys would be half brothers but the grandmother is not technically related.
