Ross Eckler, the word researcher, published an article about single-letter surnames in the early 1980s (see Related Links). He found many such examples; the surname O (common in Korea) was the most frequently occurring in phone books.

The shortest names listed by Eckler in his article are Ly O of Wichita, KS, and Su O of Trenton, NJ.

They're pretty short. Though the shortest surname is arguably I (such as Jesse I of Denver, CO) as it's the narrowest single letter...

(Footnote: Many websites report that the so-called "British Institute of Names" found that X O is the shortest name and Bithurasdinhournimlousgon Kleslinfarjilpourginjdesher (first name shortened to Bith) is the longest name. However, the source for this appears to be Bongonews, a spoof-news website. So don't believe it. It's simply not true. There is no 'British Institute of Names', anyway, and neither name makes sense in the languages they purport to come from - while 'O' is a common Korean surname, 'X' does not exist in the language, for instance.)

Update July 29, 2017: For the shortest nickname, I know someone named Dajerian can be called DJ.

"Ng" is a common Vietnamese name, and I have heard of an old Korean fisherman who claimed his name was "X O", but I have heard of no real names that were shorter.
