That depends on who is to use the toilet. children's toilet seats (in schools for example) are much lower than in an apartment and old peoples homes have even higher seats.

The Swedish standard toilet height (to the rim not including any seat) is 420 mm.

the average height of a toilet is flush with the floor. you dont raise a toilet off the floor.

The standard height for a toilet seat is 16 inches. An invalid toilet seat is 18 inches (in Canada).

16 to 18 inches off the finished floor. It depends, higher ones are good for knees, called "right height" those are 18. older toilets tend to be 16.

I would install a toilet roll holder around 18" up off the floor. It should be around the height of your legs (or a bit higher) when sitting on the 'ol throne.
