A medical physician such as a Doctor of Medicine (M.D.) or a Doctor of Osteopathic Medicine (D.O.) who is a board certified pathologist or board-certified infectious disease specialist.
There are some board certified A&I's (allergists & immunologists) who study rare diseases.
In practice, many doctors of many certifications study rare diseases, especially rare diseases within their certification or field of interest. This would include dentists (who are doctors of dental surgery or doctors of medical dentistry, depending on the dental school from which they graduated). This would also include veterinarians (animals other than humans).
Also, board certified physicians in the field of general practice, internal medicine, psychiatry, emergency medicine, pediatrics, radiologist, etc. and across the board might have the vocational and research interest to study rare diseases.
Other "doctors", too, study rare diseases, include PhD graduates, pharmacists (who actually have a doctorate in pharmacy, called a Pharm.D.--yet are not referred to as "doctors" to avoid confusions).
Rare disease is the disease that only affects small percentage of population. Example of this is rare forms of spleen cancer. Pharmaceutical company get tax deduction for doing research for such disease.

Pathology is the study of diseases.
Dr. House is a fictional charactor...there is no doctor that just does rare diseases, nor is there a hospital that gets as many of these "rare" cases as does the hospital on the show...if there was, these diseases wouldn't be rare. Dr. House is for the most part...a pathologist...these doctors look run tests and study xrays, symptoms and lab results to diagnose diseases. Much of their time is spent in a lab under a microscope...and have limited patient contact. They do not treat patients, as he does in the show...if it is something serious, they refer the patient to specialists. They also do not perform surgery...as he does in the show. Dr. House is a fictional character that is a combination of about 100 different types of doctors...if you are looking for a position that will allow you to do what House does, it does not exist...and if it did, it would take about 30-40 years of medical school...and by then no hospital would be able to afford you...but the closest thing to it would be pathologist...if you want to diagnose diseases and aid in the development of treatments and cures...while at the same time pulling in about 300-350k a year...that's where ya go
