What is the text of the oath of office of the President of the United States?

"I, [President's Name] do solemnly swear, that I will faithfully execute the office of President of the United States, and will, to the best of my ability, preserve protect and defend the Constitution of the United States, so help me G-d."

The constitution does not include the last statement "So help me G-d." This was was supposedly added by George Washington after he was sworn in as the first President in 1789.
